Select the "Available Style" and click Modify.

It’s a Windows thing:
In the Properties menu, click the center button that says “Change Icon…” If the icon does not show up, click the “Browse” button and go to the file location where the ICO file is saved. Once it is on the screen, click the icon of your choice and press “OK” Press “OK” on the Properties dialog box

R&D looked at your files but they were too corrupt to be recovered.

To reset the hotkeys:
Close IronCAD
Go to C:\Users\%USERNAME%\AppData\Local\IronCAD\IRONCAD\25.0\en-us\CustomizationDelete all XML files there.
Try IronCAD again.

I thought you said the S key sometimes brings up the old UI, I didn't see that in your video. It looked like you got the old UI from the right-click menu.
There is no way to disable that right-click menu.
